To access your medical records, select sharing in the MYUFHealth menu. Once you send in your request, your records will be released within 96 hours.

Your privacy

Due to state and federal privacy laws, only the patient or the patient’s legal representative can obtain medical information. Please be prepared to show proof of such.

UF Health Shands Hospital and UF Health Physicians

To obtain a copy of your medical records

Complete a UF Health Shands medical request form - you can call our office at (352) 594-0909 to have the form mailed or faxed. Then follow one of these options:

Non-UF Health Shands doctor’s offices

If the request is for a non-UF Health Shands doctor’s office/facility, please ask the doctor’s office to do the following:

• Use their office fax cover sheet
• Write your name, date of birth and what they need on the office fax cover sheet
• Write STAT at the top of the page
• Fax the form to the ROI office at (352) 265-1098
• The ROI office will fax the records directly to your doctor’s office
• Please contact the HIM Department at (352) 594-0909 or (352) 265-0131 with questions.

UF Health Central Florida

To obtain a copy of your medical records

Complete a PHI Release form and follow one of these options:

  Record Access Fees

  • You may review your medical record by appointment only. The review fee is $25 per hour.
  • Under Florida law you may be charged a fee of up to $1 for every page copied (plus tax and handling).
  • You can save money by asking for specific documents or a record abstract (all recent major reports) rather than the entire record.
  • The copying fee is waived for copies provided directly to a healthcare provider for continuing medical care.
  • Most record requests are fulfilled within five to seven business days.
